Documents Required to obtain Birth cetificate in kerala

Parent Name
Date of birth and location.
Application made in the format provided by government of Kerala.
If you are born in dispensary/hospital than your record on the dispensary/hospital register is enough.
Address proof of parent(Voter ID, Ration card, Drivery Licence etc).
Aadhar Card of parents.
If birth took place in home then inform the local body in writing and get the certificate.
Hospital discharge certificate.
An affidavit is required in case you have delayed the registration from judicial magistrate and a notary is required having date birth, Name of parent and evidence of event of birth.
Permission will be required from Add.Dist.Registrar i.e BDO of concerned Taluka if you have applied after a month.
If applied after one year, order from executive magistrate will be required and same should be attached in the application form.
If there is no record for birth / death from hospital, then the village head / regional councilor / MLA / MP / MBS Certificate of signature and stamp with any one of the doctor should be submitted

You will be navigated to Home page of sevana portal.
On the Home page click on the Registration for Birth and Death.
Then on that page select the form option.
Then form no-2 death report – Click here to get direct report form
You will be navigated to pdf format of death report form ,just download it and take printout of the form.
After taking the print out fill application form in hard copy.
Form contains your private information like- Date of Death, Deceased Full Name,Permanent address of deceased , Name of father/husband ,Name of mother, Address at the time of death,Sex of the deceased (Male/female) ,Age of deceased ,Place of death(Hospital/Institution-Name,House address,Other place) ,Informants Name and address ,Counter signature and seal of the authorities concerned (in the case of hospitals / institutions) and other information to be filled.

Click on the link of Registration of marriage in the bottom of the sevana civil registration web portal.
You will be navigated to page where you will get three option
- Hindu marriage act.
- Common marriage rules.
- Special marriage Act.
Click on the form from the category you want to register for.
Fill the form with date of marriage,place of marriage ,Photos of husband and wife and other details asked in the form.
